Cone beam computed tomography (CBCT) is one of the most significant advances in endodontics in the last decade. When used appropriately, it can greatly enhance the diagnosis and treatment planning of a variety of endodontic cases. However, it is important that each case is considered individually to ensure that the benefits provided by the scan outweigh the risks of the additional radiation.
The applications of CBCT in endodontic:
1-Diagnosis of vertical root fracture.
CBCT has a success rate of 90% in detecting VRF while 2D radiography has 42% success rate .
2-Diagnosis of periapical pathology.
Studies showed that CBCT is more accurate in detecting periapical pathology than 2D radiographic image .
3-Dentoalveolar trauma.
4-Assessment of complex root canal anatomy.
2D image of periapical radiograph doesn’t reveal the the real root canal numbers . but cbct can reveal the actual number and morphology of all canals beside lateral canals so give you avaluable information about the root canal complex of each tooth that help you in treatment plan.
5-Assessment of treatment complications.
Studies showed that 79% success rate of root canal treatment that has been assessed by 2D radiography has been decreased to 35% when assessed by CBCT.
6-Assessment of root resorption.
CBCT imaging not only allows for early detection of root resorption (external or internal) as compared to 2D imaging, it can also identify the extent of a lesion. Depiction of pulpal extensions in talon cusps and the localization of broken instruments are simplified by CBCT images. Assessment of biomechanical preparation of root canals using various systems by CBCT images is practicable because of its reliability and precision.
